About Our Client:
SAFREST is dedicated to enhancing the quality of life for our elderly population in Reading, PA.
Position Overview:
As a licensed Occupational Therapist, you will play a vital role in evaluating and reassessing participants, developing tailored treatment plans, and providing therapeutic interventions to help them maintain their independence at home.
You will collaborate with a supportive team, ensuring a comprehensive approach to participant care.
Key Responsibilities:
Conduct evaluations and reassessments upon referrals from the Interdisciplinary team.
Develop individualized treatment plans focused on enhancing participants' functional abilities.
Create and implement therapeutic exercise programs and assess progress toward goals.
Perform home safety assessments for new participants and update as needed.
Complete care plans during assessments, including enrollments, semi-annual reviews, and as requested by families or caregivers.
Evaluate participants' home environments for safety and equipment needs, providing in-home assessments when necessary.
Qualifications:
Valid Occupational Therapy license in Pennsylvania.
Minimum of 1 year of experience working with the frail or elderly population.
Demonstrated clinical competency with geriatric clients.
Management experience in rehabilitation or related fields is a plus.
Valid driver's license and reliable transportation.
