As a Life Skills Clinician with Community Health Network, you will play a vital role in providing care coordination and life skills training to individuals with mental health and/or substance use disorders.
Key Responsibilities- Care Team Collaboration: Work as part of a multidisciplinary care team to provide comprehensive psychosocial assessments and develop individualized care plans.
- Life Skills Training: Deliver life skills training and development to individuals, focusing on skills such as problem-solving, communication, and daily living skills.
- Documentation and Reporting: Accurately document services provided in the Electronic Medical Record (EMR) and complete necessary reports and paperwork.
- Transportation: Transport clients in personal vehicles, as needed.
- Education: Bachelor's Degree in mental health or a related field.
- Experience: Two years of social service experience in a volunteer or paid role.
- Licensure: Current Indiana Driver's License and a reliable vehicle.
