Key Responsibilities:

  • Collaborate with Neurologists and clinical personnel to create personalized healthcare plans for patients with chronic neurological disorders such as epilepsy, Parkinson's disease, and multiple sclerosis.
  • Educate and support patients and their families regarding disease management, treatment alternatives, and lifestyle changes to boost self-management skills.
  • Utilize the Chronic Care Management platform to assist multiple chronic disease patients, coordinating care transitions and ensuring seamless continuity of care.
  • Conduct thorough assessments of patients' neurological health, focusing on symptom management, medication adherence, and functional capabilities.
  • Provide healthcare teams with essential data to address the health needs of patients effectively.
  • Assist practice staff in developing proactive interventions for target populations.
  • Participate in regular meetings with colleagues and practice staff, as well as the support team.
  • Adhere to all applicable federal, state, local, and company policies and procedures in the execution of job functions.

Required Qualifications:

  • Registered Nurse with a minimum of 5 years of experience, preferably in a clinical environment.
  • In-depth knowledge of neurological conditions, treatments, and evidence-based practices in chronic disease management.
  • Proven ability to work autonomously, prioritize responsibilities, and manage time effectively in a dynamic setting.
  • Exceptional interpersonal skills with the capacity to communicate effectively with patients, families, and healthcare team members.
  • Familiarity with EMR/EHR systems.

Preferred Qualifications:

  • Previous experience in Care Management, specifically with patients facing chronic conditions.
