We are in search of a Registered Nurse Care Manager who thrives in a dynamic environment.
In this role, you will evaluate, strategize, execute, oversee, and assess the necessary options and services to fulfill an individual's healthcare requirements through a collaborative methodology.
Your responsibilities will also include delivering comprehensive ongoing case management services to patients by orchestrating and overseeing their care to address various service needs throughout the healthcare continuum.
You will ensure optimal patient outcomes that focus on quality, service, customer satisfaction, and cost efficiency.The care manager will collaborate with physicians to determine care plans and allocate resources based on the patient's risk assessment, while also aiding the patient and their family in managing illness by enhancing their self-care capabilities and supporting their rights as consumers.
Key Responsibilities- Evaluates, formulates, implements, and monitors a detailed care plan through an interdisciplinary team approach in partnership with the patient and family in both internal and external environments.
- Guarantees quality and cost-effective resource utilization while facilitating access to necessary care.
- Partners with community health education and outreach organizations to promote health for children with intricate healthcare needs.
- Engages in continuous professional development.
- Prepares and analyzes outcome data to identify opportunities for improvement.
- Demonstrates a commitment to teamwork, adaptability, professional interpersonal communication, critical thinking, and customer service excellence.
- Employs effective interpersonal skills when interacting with patients, families, visitors, colleagues, and other healthcare professionals.
- Acts as an advocate for patients and families while collaborating with the healthcare team.
- Contributes to fostering a positive work atmosphere that encourages productivity, mentorship, teamwork, and collaboration.
- Participates in the orientation and training of new team members.
All employees at Texas Children's are required to be fully vaccinated against COVID-19, unless a medical or religious exemption is approved.
- Bachelor's Degree in Nursing is required or
- Associate Degree in Nursing with acceptance required or
- Current enrollment in a BSN program with a signed agreement or an associate degree in a related field accepted by the Texas Board of Nursing for obtaining and maintaining an RN license is required or
- Technical diploma in nursing with acceptance or current enrollment in a BSN program with a signed agreement is required or
- Must be currently enrolled in a nursing bridge program working towards an MSN. Verified BSN-equivalency or a signed agreement is required.
- Bachelor's in Nursing is strongly preferred.
- RN - Lic-Registered Nurses Nurse Licensure Compact is required.
- BLS - Cert-Basic Life Support from the American Heart Association is required.
- PEARS - Cert-P.E.A.R. from the American Heart Association is preferred.
- CCM - Cert-Certified Case Manager from the Commission for Case Manager Certification is preferred.
- ACM - Cert-Accredited Case Manager from the American Case Management Association is preferred.
- A minimum of 3 years of nursing experience in an acute care setting is required.
- 3 years of experience in community health, complex pediatric patient care, home care, case management, managed care, or utilization review is preferred.
