Job Title: Licensed Practical Nurse

Job Summary:

We are seeking a highly skilled and compassionate Licensed Practical Nurse (LPN) to join our team at the New York State Civil Service. As an LPN, you will provide high-quality nursing care to individuals with developmental disabilities, ensuring their physical and emotional well-being.

Key Responsibilities:
  • Monitor the health status of individuals and provide appropriate interventions.
  • Administer medications and treatments as prescribed by physicians.
  • Order medications and medical supplies as needed.
  • Review and transcribe physician's orders to ensure accurate documentation.
  • Complete semi-annual nursing summaries and attend reviews to ensure continuity of care.
  • File medical reports, track medical appointments, and maintain accurate medical records.
  • Supervise and provide direct care services, including ADLs, to individuals with developmental disabilities.
  • Provide in-service training for Direct Support Assistant Trainees to enhance their skills and knowledge.
  • Perform personal care tasks for consumers residing in group homes, including bathing, dressing, and feeding.
  • Monitor the conditions of individuals in their assigned worksite to detect changes in condition or responses to medication or therapy.
  • Identify symptoms of illnesses and changes in conditions manifested by individuals and report them to physicians or registered nurses for diagnosis and treatment planning.
  • Take vital signs, including temperatures, pulse rates, blood pressures, and respiration rates, using thermometers, manometers, and stethoscopes.
  • Administer injectable, j-tube, and g-tube medications to assigned individuals.
  • Implement and monitor g-tube and j-tube feedings, including changing g-tubes as necessary.
  • Perform catheterization orders, trach care, deep oral and pharyngeal suctioning, and other medical procedures as required.
  • Assist individuals in collecting urine, stool, and other specimens for analysis.
  • Prepare and maintain charts and other records to document treatments administered and conditions observed.
  • Perform inventory of current medical supplies and medications to determine the need for ordering new supplies or medications.
Requirements:
  • A license and current registration to practice as a practical nurse in New York State.
  • A limited permit to practice as a practical nurse in New York State.
  • An application on file for a limited permit to practice as a practical nurse in New York State.
  • A valid Driver's license.
Working Conditions:

This position requires working in a fast-paced environment with individuals with developmental disabilities. The LPN must be able to work independently and as part of a team, with minimal supervision. The position requires standing, walking, and lifting, as well as the ability to work in a variety of settings, including group homes and community-based programs.
