Job Title: Nurse Manager - Case Management

We are seeking a highly skilled and experienced Nurse Manager to lead our Case Management Department at ChristianaCare Wilmington Hospital.

Job Summary:

The successful candidate will be responsible for managing the daily operations of the Care Management Team, providing expert guidance and support to staff and leadership, and ensuring effective and efficient discharge planning services to patients in accordance with policies, procedures, and regulatory requirements.

Key Responsibilities:
  • Manage daily operations of the Care Management Team and serve as an expert resource for staff and Christiana Care leadership
  • Evaluate and provide feedback/recommendation of information technology software for comprehensive, longitudinal care management
  • Facilitate the development of care management guidelines, algorithms, and protocols
  • Develop and implement applicable care management processes, policies, and procedures
  • Prepare and review annual performance appraisals with employees
  • Develop systems and processes to effectively manage collaborative relationships with external third-party payers
  • Identify areas of potential compliance risk and develop strategies to mitigate risk in collaboration with the Compliance Officer and staff
  • Maintain strong working relationship with compliance office and staff and actively engage in all compliance activities as requested
  • Assist in the preparation of the annual operating and capital budget
  • Develop processes for the accurate capture of data to maintain a care management dashboard reflective of key performance measures
  • Collaborate with Data Informatics team to analyze data and make recommendations for program development/changes based on data analysis
  • Collaborate with key internal stakeholders to develop and implement processes, policies & procedures to manage the transition of patients effectively and efficiently in accordance with state, federal and payer standards and regulations
  • Demonstrate knowledge of local, state, and federal reimbursement guidelines
  • Coordinate and facilitate department meetings
  • Maintain a collaborative professional relationship with clinicians and administrative staff to develop, implement, and evaluate successful process outcomes
  • Collaborate with Physician Advisors as needed
Requirements:
  • Delaware RN License or compact state RN License
  • Bachelor's degree required
  • MSN or MBA preferred
  • Five years' experience in Case Management in an Acute Care hospital setting preferred
  • Three years supervisory/management experience preferred
  • All candidates must be able to maintain required documented hours of relevant continuing education related to licensure requirements
Working Conditions:

Frequent travel to multiple sites as needed

Occasional exposure to office materials

Normal office environment
