We are seeking a dedicated Licensed Vocational Nurse (LVN) to oversee the patient intake process. This role involves collaborating with the intake team, fostering positive relationships with clients and referral sources, addressing customer inquiries, and managing insurance verification and authorization.
Key Responsibilities:- Assess hospitalized patients referred for home care services.
- Secure a complete copy of the patient's medical history.
- Verify insurance coverage for home health care services.
- Document prior authorization for home care services from insurance providers.
- Assist in obtaining managed care orders under the guidance of the HMO Intake Manager.
- Negotiate payment rates for bundled home care services.
- Work with hospital discharge planning personnel and insurance case managers to ensure safe and timely discharges.
- Consult with healthcare professionals to develop a coordinated home care plan.
- Verify completion of required face-to-face assessments.
- Engage with patients, families, and caregivers to discuss home care needs and relevant psychosocial factors.
- Complete referral documentation including intake data and care plans.
- Maintain relationships with hospital and insurance personnel, providing education on services and coverage.
- Participate in patient care conferences and training sessions.
- Collect and maintain statistical data on referrals.
- Attend meetings and contribute to committees as necessary.
- Notify management of any delays in clinical care.
- Perform additional duties as assigned by management.
- Graduate of an accredited practical nurse or vocational nursing program.
- Minimum of three years of nursing experience, with at least one year in a community/home health or medical/surgical setting.
- Current licensure as a practical nurse or vocational nurse in the state.
- CPR certification required.
- Proficient in medical terminology, office procedures, and data entry.
- Strong interpersonal and communication skills.
- Ability to establish effective working relationships.
- Valid driver's license and personal vehicle for travel.
