About the Role

We are seeking a highly skilled Clinical Manager to lead our Advanced Illness Management (AIM) Palliative Care program. As a key member of our team, you will be responsible for coordinating and managing all day-to-day business affairs and clinical activities related to the administration of the AIM and Palliative Care department.

Key Responsibilities
  • Clinical Care Delivery: Ensure that clinical care delivery goals are met and exceeded, driving exceptional patient outcomes and satisfaction.
  • Team Management: Manage a team of AIM Clinical Coordinators, AIM NPs, and AIM RNs, providing guidance, support, and development opportunities to ensure their success.
  • Operational Efficiency: Ensure scheduling, frequency adherence, clinical coordination, medical record maintenance, and marketing functions are maintained for a team of patients and clinicians, optimizing operational efficiency and productivity.
  • Communication and Collaboration: Manage incoming calls from partners and patients, ensuring timely and effective communication and collaboration to meet their needs.
  • Visit Frequency Compliance: Manage visit frequency compliance, scheduling PRN visits, post-hospitalization follow-up with patients and families, expediting discharge home with AIM communication with partners, and scheduling post-hospitalization visits.
  • Orders and Coordination: Order labs, DME, and imaging, coordinating results with NPs, entering telephone encounters in partner EMR, and entering routine visit med reconciliation in partner EMR.
  • Transitions and Referrals: Ensure transitions to HSP referrals to CTC for care type, identifying HSP eligible patients, and communicating with local partner offices for lab drop-off.
About You
  • Qualifications: Registered Nurse with Home Health, Hospice, and/or Palliative Care experience preferred. Bachelor's degree in nursing, Business Administration, or related field; or equivalent experience and knowledge in Health Care Operations required.
  • Experience: Minimum of 2 years health care administration/operations management required. Must be able to work some nights/weekends on call.
  • Certifications: Certified in Hospice and Palliative Care - preferred.
