Key Responsibilities

The Patient Care Management Assistant (LPN) plays a crucial role in supporting the Patient Care Manager by coordinating a dedicated team to ensure the delivery of high-quality care to home health patients. This position involves:

Qualifications

Experience Requirements

Licensure Requirements

  • Current CPR certification is mandatory.
  • A valid driver's license, vehicle insurance, and access to reliable transportation or public transit are essential.
