Job SummaryWe are seeking a skilled and compassionate Hospice Care Specialist to join our team. As a key member of our interdisciplinary team, you will provide high-quality care to patients and their families, working collaboratively with our team to ensure a seamless and supportive experience.
Responsibilities- Provide hands-on nursing care to patients in their homes, hospitals, or other care settings
- Participate in team meetings and contribute to the development of patient care plans
- Document patient responses, observations, and care provided
- Collaborate with interdisciplinary team members to ensure comprehensive care
- Provide emotional support and education to patients and their families
- Current certification as a Nurses' Aide with an unrestricted, active certification
- 1 year of experience as a Hospice Aide or Nurses Aide
- Interpersonal skills sufficient to interact effectively with patients, families, and team members
- Ability to learn computer skills related to clinical software and electronic clinical records
- Reliable vehicle, valid driver's license, and required automobile insurance
