Empowering Patients through Compassionate Care

A Home Health Aide plays a vital role in providing personalized care to patients in the comfort of their own homes. As a Home Health Aide with CenterWell Home Health, you will have the opportunity to make a meaningful difference in the lives of our patients.

Key Responsibilities:
  • Direct Patient Care: Provide assistance with daily living activities, such as bathing, grooming, and dressing, under the supervision of a registered nurse.
  • Medication Management: Assist patients with self-administered medications, ensuring accurate dosing and administration.
  • Health Monitoring: Take vital signs, including temperature, pulse, and blood pressure, and report any abnormal results to the supervising nurse.
  • Personal Hygiene: Help patients maintain good personal hygiene by performing or supervising bathing, grooming, and other activities.
  • Nutrition and Feeding: Assist patients with feeding, including safely positioning them for meals and observing and recording food and fluid intake.
  • Toileting and Mobility: Assist patients with toileting, including use of bed pans and urinals, and provide necessary skills to safely assist with patient mobility, exercises, and transfers.
  • Documentation and Communication: Accurately document interactions with patients, caregivers, doctors, and other staff members, and report changes and pertinent information to ensure continuity of care.
Requirements:
  • Education: High school diploma or equivalent required.
  • Certification: Completion of a Certified Nursing Assistant or Certified Home Health Aide program within the last 24 months, and meeting applicable state certification requirements.
  • Experience: At least one year of experience in the last 24 months as a Home Health Aide or Certified Nursing Assistant in a hospital, nursing home, or home health/hospice agency.
  • Transportation: A valid driver's license, auto insurance, and reliable transportation are required.
