Job Summary

We are seeking a skilled and compassionate Registered Nurse to join our team as a Clinical Admissions Nurse. In this role, you will be responsible for ensuring that our clients receive the highest level of care and support, while also maintaining strong relationships with referral sources and other healthcare professionals.

Key Responsibilities
  • Conduct initial and periodic assessments of clients to determine their care needs and develop personalized care plans.
  • Implement and evaluate the effectiveness of care plans, making adjustments as needed to ensure optimal outcomes.
  • Develop and maintain strong relationships with referral sources, including physicians, hospitals, and other healthcare professionals.
  • Collaborate with other healthcare professionals to ensure seamless care coordination and communication.
  • Provide education and support to clients and their families to promote health and well-being.
  • Maintain accurate and up-to-date records of client care, including progress notes and care plans.
  • Participate in ongoing education and training to stay current with best practices and industry developments.
Requirements
  • Current licensure as a Registered Nurse in the state of Minnesota.
  • At least two years of experience in a healthcare setting, preferably in home care or a related field.
  • Strong organizational and communication skills, with the ability to work independently and as part of a team.
  • Ability to make sound clinical judgments and prioritize tasks effectively.
  • Basic computer skills and proficiency in software applications, including Microsoft Office.
  • Understanding of Medicare certification and OASIS documentation requirements.
  • Valid driver's license and proof of car insurance.
  • Proof of negative Mantoux or documentation of negative chest X-ray.
  • Current CPR certification.
  • U.S. Citizenship or evidence of valid Alien Work Permit.
