We are seeking a highly skilled Licensed Practical Nurse (LPN) to join our team as a MAT Coordinator, providing comprehensive clinical services to members enrolled in our Medication-Assisted Treatment (MAT) program.
Key Responsibilities- Assess patients' physical, psychological, and social aspects to develop individualized treatment plans in consultation with RNs and other staff.
- Carry out treatment plans, including administering medications, obtaining diagnostic tests, and assisting in medical or minor surgical procedures as directed.
- Adhere to established policies, procedures, and protocols, including counting controlled substances, using proper safety techniques, and responding to health emergencies.
- Document nursing encounters using SOAP charting and communicate essential information to relevant healthcare staff.
- Attend mandatory staff meetings and training sessions to stay updated on healthcare practices and protocols.
- Graduate from an accredited school of nursing.
- Prefer a minimum of one (1) year clinic experience.
- Have and maintain current licensure as a Licensed Practical/Licensed Vocational Nurse within the state of employment.
- Must be able to obtain and maintain CPR certification.
