• Conduct assessments to identify family strengths and challenges
• Develop and implement family-centered service plans to address areas of concern
• Collaborate with community agencies to provide referrals and support services
• Participate in a multidisciplinary team approach to provide family-focused care
Requirements:
• Bachelor's degree in social work, human services, or a related field
• 1-2 years of experience working with families in a home setting
• Knowledge of infant and child development
• Ability to establish trusting relationships and work with diverse populations
• Excellent verbal and written communication skills
• Bilingual in English and Spanish preferred
