Job Summary

VNS Health is seeking a skilled Registered Nurse to serve as a Field Supervisor in our Home Health division. As a key member of our team, you will be responsible for providing clinical leadership and support to our Home Health Aides, ensuring that patients receive high-quality care and exceptional outcomes.

Key Responsibilities
  • Conduct regular field visits to patients' homes to assess their needs and provide training and support to Home Health Aides
  • Develop and implement individualized care plans that meet the unique needs of each patient
  • Collaborate with the clinical care team to ensure seamless care coordination and communication
  • Orient and supervise Home Health Aides in the field to ensure they have the necessary skills and knowledge to provide high-quality care
  • Document patient visits and care plans in our electronic medical record system
Requirements
  • Current license to practice as a Registered Nurse in New York State
  • Minimum of two years of experience as an RN in a home care setting
  • Valid driver's license may be required
