Job Summary:
We are seeking a compassionate and skilled Hospice Aide to provide high-quality patient care services in the comfort of patients' homes. As a key member of our care team, you will work closely with our nursing staff to deliver personalized care and support to patients and their families.
Key Responsibilities:
- Participate in patient care conferences and meetings to ensure seamless care coordination.
- Attend regular continuing education sessions to stay up-to-date on the latest care practices and regulations.
- Provide exceptional patient care, including personal hygiene, mobility assistance, and meal preparation, as directed by our nursing staff.
- Assist patients with daily activities, such as bathing, dressing, and grooming, while promoting independence and dignity.
- Collaborate with our physical therapy team to support patients' rehabilitation goals.
- Record and report patient progress, including vital signs and any changes in condition, to our nursing staff.
- Maintain accurate and detailed records of patient care, including visit reports and care plans.
- Ensure a safe and clean environment for patients and staff, adhering to infection control and safety protocols.
- Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
Requirements:
- At least one year of experience in hospice or home health care, with a strong understanding of patient care principles and practices.
- Hold a valid certificate or completion of required curriculum, with competency evaluated in accordance with Federal/State regulations.
- Demonstrate emotional maturity and excellent communication skills to establish and maintain positive relationships with patients, families, and healthcare team members.
