We are seeking a highly skilled Care Coordinator to join our team, who will work collaboratively with clients, family members, and community partners to achieve therapeutic goals.

Key Responsibilities
  • Develop and implement individualized care plans that address the unique needs of each client
  • Facilitate communication among team members and community partners to ensure effective service coordination
  • Conduct assessments to identify opportunities and barriers to access WISe services across various systems
  • Collaborate with the WISe treatment team to develop long-term goals and objectives for program services
