Nurse Liaison Specialist

- Assist in transferring patients from home health services to palliative or hospice care, ensuring uninterrupted medical care. ">
- Coordinate the transfer of patients residing in SNFs or discharging from hospitals to palliative or hospice services. ">
- Provide patient care, using various skills and necessary coordination of services. ">
- Document patient care according to policy and procedure. ">
- Manage day-to-day branch office tasks, ensuring patient care documentation is complete and submitted timely. ">
- Assist in managing patient care supplies. ">
- Assist with Medicare part D medications management. ">
- Collaborate with the Hospice Registered Nurse on medication reconciliation and pharmacist recommendations. ">
Requirements:">">
- ">
- Current LPN licensure within the state of branch location and two years of clinical nursing experience, preferably one year in Home health/Hospice setting. ">
- Ability to independently organize work and provide effective healthcare services to hospice patients. ">
- Travel and working conditions as assigned. ">
- Effective communication skills (oral and written), CPR certification, and a minimum of twelve hours of continuing education in a twelve-month period. ">
