Physical Therapist Specialist

We are seeking a skilled Physical Therapist to join our team at Sodus Rehab & Nursing Center. This is a rewarding opportunity for an experienced professional to provide high-quality care to our residents and contribute to the success of our rehabilitation department.
About the RoleThe successful candidate will have a current unencumbered NYS license as a Registered Professional Physical Therapist, with a Bachelor's degree or higher. A graduate of an accredited school of physical therapy or international equivalent is required. Geriatric experience is preferred, and a current unrestricted driver's license and dependable transportation are essential.
The ideal candidate will possess excellent communication skills, both written and verbal, and be able to work effectively in a fast-paced environment. They must also be able to use necessary computer applications, function independently, and as part of a team, and adapt to changing situations.
Responsibilities- Demonstrate working knowledge of skilled restorative therapy and maintenance programs.
- Screens residents for rehabilitation therapy needs.
- Completes Physical Therapy evaluations upon physician order.
- Establishes Physical Therapy plan of care for treatments per physician order.
- Provides Physical Therapy plan of treatments and modalities.
- Modifies treatment program level, intensity, and techniques based on the resident's response to specific treatment plans.
- Documents resident progress toward Physical Therapy goals.
- Discharges residents from Physical Therapy services when appropriate.
- Makes appropriate recommendations for nursing regarding mobility, positioning, and ROM resident needs.
- Completes Daily Activity Schedule, progress notes, and re-certifications per facility policy.
- Supervises Physical Therapy Assistants and unlicensed assistive personnel.
- Provides resident, staff, and family education and in-servicing.
- Demonstrates working knowledge of Medicare Part A PPS case management.
- Complies with Federal and State regulations related to rehabilitation therapy.
- Completes all documentation to comply with professional standards and regulatory requirements.
