Dialysis Outpatient Licensed Practical Nurse

The Licensed Practical Nurse (LPN) provides high-quality patient care and treatment in an outpatient dialysis setting. The LPN promotes patient comfort and provides emotional support to patients and their families.
Key Responsibilities- Respond to patient calls in a professional and timely manner, taking accurate messages and ensuring they are delivered to the appropriate person.
- Document telephone contacts in accordance with established policies and procedures.
- Facilitate communication and team involvement among providers, staff, patients, and service partners through timely sharing of information.
- Assist with provider assessments by gathering information according to established policies and procedures.
- Monitor patient status during specialized procedures performed by physicians/providers, gathering data pertaining to patient condition prior to, during, and after the procedure.
- Legibly document patient care information in the medical record chart in a timely manner and maintain the chart appropriately.
- Perform direct patient care within the scope of practice and willingness to cross-cover for nursing.
- Administer medication, treatment, and tests as ordered in accordance with applicable state and federal laws and policies and procedures.
- Refer telephone messages to appropriate personnel and document telephone contacts per policies and procedures.
- Competently handle phone triage for patients with critical problems or shortness of breath, documenting legibly and accurately.
- Adhere to all policies and procedures relative to the organization, including safety regulations regarding equipment, facility, and hazardous materials.
- Act as a patient advocate in the coordination and delivery of quality patient care, consistently attempting to meet patients' expectations for service and providing for patient comfort and safety during visits.
- Complete patient teaching using established protocols as required.
- Maintain a clean and safe environment and participate in overall quality management activities.
- Respect patients' rights, privacy, and confidentiality at all times and remain courteous, helpful, and professional.
- Maintain optimum safety standards for all patients.
- Pursue professional development opportunities and contribute to support staff development and training.

Education and ExperienceHigh School Diploma required.
Must possess current LPN license.
4 years' related experience.

Proficient with PCs and Microsoft Office applications.
Hourly Rate$39.00
