Community Support Facilitator

We are seeking a highly motivated and compassionate Community Support Facilitator to join our team at Hope Services. As a Community Support Facilitator, you will play a vital role in supporting individuals with developmental disabilities and mental health needs to participate in activities and receive services in community settings.
Key Responsibilities:- Provide individualized support to consumers in community settings, promoting their independence and autonomy.
- Develop and implement person-centered plans to meet the unique needs and goals of each consumer.
- Facilitate community integration by connecting consumers with community resources and activities.
- Collaborate with consumers, family members, and other stakeholders to ensure seamless support and services.
- Document and maintain accurate records of consumer progress and services provided.
- High school diploma or equivalent required; related experience or education preferred.
- Valid California driver's license and clear driving record required.
- Ability to work independently and as part of a team.
- Excellent communication and documentation skills.
- Knowledge of alternative communication systems and assistive technology.
- CPR and First Aid certification (training provided if needed).
- Competitive hourly rate of $21.25.
