Optum Home & Community Care is creating a new model of care that addresses the physical, mental, and social needs of patients. As a team member of our Senior Community Care (SCC) product, you will work with a team to provide care to patients at home, in nursing homes, or assisted living facilities.

Key Responsibilities:

  • Conduct comprehensive assessments of patients
  • Manage members through acute medical situations or exacerbations of chronic conditions
  • Ensure accurate and complete documentation, including ICD-10 conditions
  • Communicate and collaborate with the interdisciplinary care team
  • Provide patients and caregivers with counseling and education

Requirements:

  • Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program
  • Active and unrestricted license in the state of Alabama
  • Ability to obtain post-hire or have current and active DEA licensure/prescriptive authority
  • Ability to complete physical requirements of the job, including lifting a 30-pound bag and navigating stairs
  • Access to reliable transportation for travel to assigned facilities in Birmingham

Preferred Qualifications:

  • 1+ years of clinical experience in practice, preferably in a long-term care setting
  • Experience in meeting the medical needs of patients with complex behavioral, social, and/or functional needs
