Key Responsibilities:
- Oversee the medication use process in a designated clinical area, ensuring safe and effective pharmacotherapy.
- Engage in daily patient care rounds, offering drug information and consultations to healthcare professionals.
- Assess drug therapy for appropriateness, monitoring for effectiveness and adverse effects.
- Maintain accurate medication profiles and ensure proper medication reconciliation.
- Educate healthcare staff on current drug-related policies and procedures.
- Identify and report adverse drug events, contributing to quality improvement initiatives.
- Participate in emergency response activities as needed.
- Provide patient education during discharge planning.
- Identify opportunities for cost-effective medication use.
- Contribute to multidisciplinary committees focused on pharmacy and therapeutic practices.
Education and Experience:
- Required: Bachelor of Science in Pharmacy or Doctor of Pharmacy with relevant hospital experience.
- Preferred: Completion of PGY2 residency and board certification in a specialized area.
Work Schedule:
