About the Role

We are seeking an experienced Senior Hospice Nurse - On Call Specialist to join our team. As a key member of our interdisciplinary team, you will provide direct patient care services to hospice patients and supportive care to their families after normal business hours on an on-call basis.


Key Responsibilities
  • Provide direct patient care services to hospice patients and supportive care to their families after normal business hours on an on-call basis.
  • Manage an interdisciplinary group consisting of Licensed Practical/Vocational Nurses, Certified Nursing Assistants, and support staff to enhance the quality of hospice care.
  • Evaluate patient/family needs and assist in the preparation and execution of the plan of care for each patient.
  • Maintain all assigned patient clinical records.
  • Participate in company quality assessment and performance improvement programs.

About You
  • You must be a compassionate, kind, and empathetic individual who can work effectively with patients and their families.
  • A current RN license (in the state of requested employment or ability to obtain) is required.
  • At least 1+ years of nursing experience in hospice, home health, or oncology is preferred.
