Expert Hospice Admissions Nurse

About the RoleWe are seeking a skilled Registered Nurse to join our admissions team. In this critical position, you will be responsible for assessing patients' needs, explaining our services, and ensuring seamless transitions into our care.
You will report directly to the Admissions Manager and work closely with other healthcare professionals to provide comprehensive care to patients and their loved ones.
Key Responsibilities- Conduct thorough assessments of patients' needs and develop personalized plans of care.
- Communicate effectively with patients, families, and healthcare teams to ensure smooth transitions and optimal outcomes.
- Maintain accurate records and documentation, adhering to regulatory requirements and company policies.
To succeed in this role, you must possess:
- A current Registered Nurse license in the state where the program is located.
- A Bachelor's Degree in Nursing (BSN) or higher.
- At least one year of general nursing experience, preferably in hospice care.
