Job SummaryWe are seeking a skilled Nurse Practitioner or Physician Assistant to join our team as a Home Risk Assessment Specialist. In this role, you will conduct comprehensive health assessments in patients' homes, identifying potential health risks and developing personalized care plans to improve their overall health and well-being.
Responsibilities- Conduct thorough health assessments in patients' homes, including medical history, physical examination, and review of medications
- Identify potential health risks and develop personalized care plans to address these risks
- Collaborate with patients, families, and healthcare providers to implement care plans and improve health outcomes
- Provide education and support to patients and families to promote healthy behaviors and self-management of chronic conditions
- Document all assessments and care plans in a timely and accurate manner
- Masters degree in Nursing or Physician Assistant studies
- Current RN and NP or PA licensure in the state of practice
- Board certification in a relevant specialty
- Minimum of 1 year experience as a Nurse Practitioner or Physician Assistant
- Strong communication and interpersonal skills
- Ability to travel to patients' homes and work in a variety of settings
