Job Summary

We are seeking a highly skilled and compassionate Social Work Professional to join our team at The Laurels of Goshen. As a Social Work Professional, you will play a vital role in providing medically-related social services to our guests, helping them to attain and/or maintain their physical, mental, and psychosocial well-being.

Responsibilities
  • Admissions and Social Services Evaluations: Complete thorough assessments of guests' needs and develop personalized care plans to ensure their well-being.
  • Financial and Legal Support: Assist guests with financial and legal matters, such as applying for pensions, referrals to lawyers, and preplanning arrangements with funeral homes.
  • Discharge Planning: Coordinate with guests, families, and external agencies to ensure safe and successful discharges from our facility.
  • Counseling and Psychiatric Services: Provide or arrange needed counseling services and psychiatric medication management as indicated.
  • Documentation and Record-Keeping: Maintain accurate and timely documentation in MDS care plans.
Qualifications
  • Education and Licensure: Bachelor's degree in social work, psychology, or a related field required. Current social work licensure in the state preferred but not required.
  • Experience: One year of social work experience in a skilled nursing facility setting preferred but not required.
