Atria Physician Practice New York PC is a dynamic and innovative membership-based preventive health care practice delivering cutting-edge primary and specialty care. We are seeking an exceptional Advanced Care Provider to join our expanding Internal Medicine team. This is a fantastic opportunity to work closely with our Chief Medical Officers and contribute to providing high-quality patient care in a fast-paced environment.
Job Summary
The Advanced Care Provider will be responsible for performing comprehensive health assessments, diagnosing acute and chronic health conditions, and developing appropriate treatment plans. You will collaborate with our interdisciplinary healthcare teams to coordinate patient care effectively and provide proactive, preventive, and precision-based care.
Key Responsibilities:
- Patient Assessment and Diagnosis: Conduct thorough medical history reviews, physical examinations, and diagnostic tests to identify patients' health status and needs.
- Treatment and Management: Collaborate with Chief Medical Officers to provide primary and specialty care services, including prescribing medications, ordering, and interpreting diagnostic tests.
- Health Promotion and Disease Prevention: Educate patients and families about health promotion strategies, disease prevention measures, and lifestyle modifications to improve overall health and well-being.
- Clinical Care: Perform clinical skills such as phlebotomy, IV insertion, and EKG.
- Documentation and Recordkeeping: Maintain accurate and up-to-date medical records, ensuring compliance with legal and regulatory requirements.
Requirements:
- NYS License: Current NYS License required as NP or PA-C
- Experience: 2+ years of experience as a practicing NP or PA-C
- Certifications: BLS & ACLS certification from the American Heart Association
- Work Schedule: Availability to work Monday through Friday on a 4x10 schedule with weekend coverage as needed
