We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at Aveanna. As a Private Duty Nurse LPN, you will provide high-quality patient care in the comfort of their own homes, working closely with our interdisciplinary team to deliver exceptional outcomes.
Job Summary:The successful candidate will be responsible for delivering and documenting skilled nursing care, under the supervision of a Registered Nurse, in accordance with the developed care plan and physician orders. This role requires a strong attention to detail, excellent communication skills, and the ability to work independently in a fast-paced environment.
Key Responsibilities:- Deliver and coordinate quality patient care in compliance with physician orders.
- Continuously observe and assess patient condition and care needs, reporting changes to the supervisor and/or physician as appropriate.
- Document all activities, assessments, nursing actions, responses, and coordination of care in a timely manner.
- Participate in and update the nursing care plan.
- Take appropriate nursing action based on assessment and achieve expected outcomes.
- Graduate of an accredited school of nursing.
- Current, unrestricted state license as a Licensed Nurse in the state of practice.
- Current CPR certification.
- Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures.
- Six (6) months of recent experience as a Licensed Nurse in a clinical care setting.
- Home health experience.
- Must be able to speak, write, read, and understand English.
- Must be able to travel.
- Must be able to lift 50 pounds.
- Must be able to sufficiently reposition patients and move equipment without assistance.
- Must be able to function in a wide variety of environments, which may involve exposure to allergens and other various conditions.
- Possible exposure to blood, bodily fluids, and infectious diseases.
