We are seeking a compassionate Nurse Practitioner to join our team at Abode Care Partners.

Job Overview

The House Calls Nurse Practitioner provides medical care to seniors and patients with complex needs in various settings. This role involves examining and treating chronic and urgent illnesses, interpreting diagnostic tests, and collaborating with other healthcare professionals to ensure comprehensive patient care.

About the Role
  • Perform evidence-based medical care by planning, directing, and evaluating patient medical needs
  • Examine and treat chronic and urgent episodic illnesses, minor injuries, accidents, and other injuries or illnesses
  • Order and interpret laboratory and diagnostic testing as needed
  • Collaborate with caregivers and other healthcare professionals in providing interdisciplinary care and maintaining a comprehensive plan of care
  • Provide referrals to other healthcare practitioners when necessary
