ROLE SUMMARY:
This position offers a unique opportunity for a physician to spearhead an innovative geriatric service line. The role encompasses both direct patient care and consultative/educational support for local healthcare providers. Key responsibilities include:
- Designing and implementing geriatric programs
- Providing clinical care and oversight of operations
- Building and maintaining relationships with stakeholders
Additionally, our organization offers specialized medical services to several Skilled Nursing and Subacute Rehabilitation facilities. This role will involve providing consultative support to the independent SNF service line, ensuring a seamless integration of services while maintaining distinct operational identities. The position reports directly to the Chief Medical Officer.
QUALIFICATIONS:
- Graduation from an accredited medical school and completion of a Family Medicine or Internal Medicine residency
- Board certification in Internal Medicine or Family Medicine
- Geriatric fellowship and board certification is preferred but not mandatory
- Eligibility for licensure in the State of Vermont
- Experience working with the geriatric population
- Strong communication skills
