Job Summary:

To provide high-quality nursing care to terminally ill patients in their homes, focusing on comfort, dignity, and quality of life. Collaborate with the interdisciplinary team to develop and implement individualized care plans, ensuring seamless coordination of care.

Key Responsibilities:
  • Assess and reassess patients' physical and emotional needs, identifying areas for improvement and implementing interventions as needed.
  • Develop and maintain accurate, comprehensive patient records, ensuring timely and effective communication with the healthcare team.
  • Provide education and support to patients and their families, empowering them to make informed decisions about their care.
  • Collaborate with the interdisciplinary team to coordinate care, ensuring continuity and consistency in patient care.
  • Participate in on-call rotations, providing 24/7 coverage and responding to patient needs in a timely and compassionate manner.
  • Maintain up-to-date knowledge and skills in end-of-life care, palliative care, and hospice services, ensuring best practices are implemented in patient care.
Requirements:
  • Current RN licensure in the state of practice.
  • Minimum 2 years of experience as a registered nurse, with a focus on hospice or palliative care.
  • Active patient contact within the past three years, with a strong understanding of patient-centered care.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Ability to work independently, with minimal supervision, and make sound judgments in high-pressure situations.
Working Conditions:

Work in patients' homes, with occasional visits to healthcare facilities or hospice centers. Must be able to tolerate exposure to elements such as odors, blood, and body fluids, as well as adverse environmental conditions and hazardous materials.

Transportation:

Must have a current valid driver's license, auto liability insurance, and reliable transportation.

Essential Functions:

Regularly assesses and reassesses the nursing needs of the Hospice patient. Provides dietary counseling. Provides Hospice nursing services, treatments, and preventive procedures. Initiates nursing procedures appropriate for the patient's Hospice care and safety. Observes signs and symptoms and reports to the physician and IDG members any unexpected changes in the patient's physical or emotional condition.
