Hospice Liaison

Amedisys, a leading home health and hospice provider, seeks a skilled Hospice Liaison to join our team. As a key member of our business development team, you will be responsible for building relationships with healthcare professionals, patients, and families to promote our home care services.

Key Responsibilities:

• Educate healthcare professionals, patients, and families about home care services
• Develop and maintain relationships with key referral sources, such as hospitals, nursing homes, and physician offices
• Coordinate patient admissions with the team, including obtaining required documentation
• Conduct on-site visits to assess patient needs and identify barriers to care
• Keep accurate records of patient interactions and referrals
• Achieve admissions, product, and payor-mix goals
• Meet short and long-term target account goals

Requirements:

• At least one year of experience in a healthcare-related business development/outreach role
• Reliable transportation, current driver's license, and liability insurance
