Job Overview

This position offers a Full-Time role within our outpatient rehabilitation services.


Key Responsibilities

  • Conduct comprehensive evaluations of patients' abilities in daily living, recreation, education, and social interactions.
  • Design and implement tailored therapeutic plans based on thorough patient assessments and physician directives.
  • Monitor and document patient progress, making necessary adjustments to treatment plans as required.
  • Administer various therapy modalities to enhance patient recovery.
  • Collaborate with a multidisciplinary team to ensure cohesive care delivery.
  • Educate patients and their families about health conditions, treatment options, and strategies for health promotion and medication management.
  • Assess patients' progress and readiness for discharge, determining the need for additional services.
  • Maintain accurate records of evaluations, treatment plans, and patient progress in the medical documentation system.

Qualifications

Education:

  • Bachelor's degree in Physical Therapy is required; a Master's degree is preferred.

Work Experience:

  • Experience working with outpatient adult and pediatric populations is preferred.

Licenses and Certifications:

  • Current Physical Therapy license in New Jersey is required.

Required Skills:

Exceptional customer service abilities are essential.
