About the Job

We are seeking a compassionate and skilled Medical Social Worker to join our team at Saint Anthony Hospital. In this role, you will provide essential social work services to adult psychiatric patients, ensuring they receive the support and care they need to recover and thrive.

Job Summary

As a Medical Social Worker, you will conduct psychosocial assessments of patients and their families, gathering valuable information to inform discharge planning and provide emotional support. You will also collaborate with patients, family members, and healthcare providers to develop comprehensive post-hospital care plans, addressing the unique needs of each individual.

Essential Responsibilities

  1. Patient Assessments and Discharge Planning: Conduct thorough psychosocial assessments of patients and their families to obtain information about home environments, family relationships, and coping resources. Use this information to develop discharge plans that meet the unique needs of each patient.
  2. Education and Support: Educate patients and their families about their illnesses and treatment plans, promoting understanding and acceptance. Gather pertinent information from medical records, hospital personnel, and significant others to inform discharge planning and post-hospital care.
  3. Communication and Collaboration: Communicate patient and family issues to appropriate staff members and attending physicians. Participate in team and family conferences to ensure patient understanding and compliance with medical plans.
  4. Discharge Planning and Coordination: Collaborate with patients, family members, and healthcare providers to develop and implement discharge plans, addressing needs such as nursing home placement, rehabilitation, and transportation.
  5. Abuse and Neglect Response: Assess and intervene in cases of abuse and neglect, preparing and maintaining required documentation.
  6. Advance Directives Program: Participate in the Advance Directives Program as a liaison, providing patients with information and support regarding advance directives.
  7. Group Therapy Leadership: Lead weekly group therapy sessions on the inpatient unit, promoting patient engagement and support.

Marginal Responsibilities

  • Job Safety and Quality: Perform essential and marginal job functions in a safe manner, adhering to hospital and departmental standards and confidentiality norms.
  • Performance Improvement: Participate in performance activities, including problem identification, data collection, solution selection, implementation, and evaluation.
  • Staff Meetings and Training: Participate in staff meetings and other inservice meetings or committees as required.

Job Requirements

  • Excellent Observation and Communication Skills: Require excellent observation skills, analytical thinking, problem-solving, and ability to communicate and interact with all levels of professionals.
  • Critical Thinking and Teaching Skills: Good critical thinking skills; ability to assess, evaluate, and teach. Flexible with a working knowledge of all areas of medicine.
  • Prioritization and Organization Skills: Ability to adjust priorities quickly, organize multiple tasks simultaneously.
