About the Role

We are seeking a compassionate and skilled Medical Social Worker to join our team at Agape Care Group. As a Medical Social Worker, you will play a vital role in providing emotional support and psychosocial evaluations to patients and their families during their end-of-life journey.

Key Responsibilities
  • Conduct thorough psychosocial evaluations to identify patients' and families' needs and develop personalized plans of care.
  • Provide emotional support and counseling to patients and families, addressing their concerns and fears.
  • Collaborate with healthcare teams to ensure comprehensive care and coordination of services.
  • Develop and maintain relationships with patients, families, and healthcare providers to ensure continuity of care.
