We are seeking a highly skilled Medical Social Worker to join our team at Oak Street Health. As a Medical Social Worker, you will play a critical role in providing comprehensive care coordination services to our patients, ensuring they receive the best possible care and support.
Key Responsibilities- Proactively identify and address the psychosocial needs of our patients, providing assessments, advocacy, and support to help them achieve their health goals.
- Collaborate with our interdisciplinary care team to develop and implement comprehensive care plans that address the unique needs of each patient.
- Provide education and support to patients and their families on disease management, health literacy, and wellness.
- Develop and maintain relationships with community resources and organizations to ensure patients have access to the support they need.
- Utilize data and metrics to inform practice and drive outcomes, ensuring the highest level of quality care for our patients.
- Participate in quality improvement initiatives and contribute to the development of best practices in care coordination.
- Master of Social Work degree from a school of social work accredited by the Council on Social Work Education (CSWE).
- Minimally licensed in the state where OSH Center is located.
- Fluency in Spanish, Polish, Russian, or other languages spoken by people in the communities we serve (where necessary).
- Proficient computer skills, including Microsoft, GSuite, and electronic medical records.
- US work authorization.
- Prior experience with complex care coordination and/or population healthcare management.
- Familiarity with using metrics and data to guide practice and drive outcomes.
- Minimum one year of medical social work experience in a healthcare environment, preferably within the last two years.
