Job Summary

We are seeking a skilled Registered Nurse to join our team at ChristianaCare. As a key member of our Primary Care Practice, you will provide high-quality patient care and support our team of healthcare professionals.

Key Responsibilities

* Provide phone triage support for patients and ensure appropriate disposition
* Perform Nurse Visits, including Annual Wellness Visits, Advance Care Planning, and Transitional Care Management calls
* Conduct point of care tests, EKGs, and other tests as directed by clinicians
* Assist providers in examinations and procedures
* Collaborate with team members to provide a smooth and efficient flow of patients

Requirements

* Graduate of a state-approved or National League of Nursing accredited School of Nursing
* RN license or eligible for licensure in the state of Delaware and Pennsylvania
* Two years' experience as a RN, physician office experience preferred
