About the Role:
We are seeking a skilled and compassionate Home Health Certified Occupational Therapist Assistant to join our team. As an Occupational Therapist Assistant, you will work one-on-one with patients to empower them to live safely and productively in their homes.
Key Responsibilities:
- Work with patients to adapt their environment, modify tasks, and use equipment to improve participation in daily activities and quality of life.
- Collaborate with patients and families to develop personalized care plans that meet their unique needs and goals.
- Provide therapy services to patients in their homes, including assessment, treatment, and education.
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
Requirements:
- Graduate of an accredited school of occupational therapy assistant with an Associate Degree in Occupational Therapy.
- Current State Certification as an Occupational Therapist Assistant.
- Valid Driver's License.
- Current CPR, negative TB screen, and Hepatitis consent/declination.
- One year experience as an Occupational Therapist Assistant in an acute care setting.
