About the Role

We are seeking a highly skilled and compassionate Senior Patient Care Manager to join our team at Heartland Hospice. As a key member of our leadership team, you will be responsible for overseeing the day-to-day operations of our hospice sites, ensuring the delivery of high-quality patient care, and managing a team of healthcare professionals.

Key Responsibilities
  • Supervise and manage the performance of assigned team members, ensuring they meet the highest standards of patient care and service.
  • Administer the clinical aspects of the hospice program, including managing patient admissions, evaluations, and care plans.
  • Oversee the management of clinical and administrative functions, including recordkeeping and compliance with regulatory guidelines.
  • Respond to phone referrals and inquiries in a timely and professional manner, providing compassionate support to patients and their families.
  • Participate in on-call nursing schedules and provide direct skilled bedside nursing care as needed.
Requirements

To be successful in this role, you will need:

  • A degree in nursing and a current license to practice in the state of employment.
  • At least three years of experience in direct patient care, with a minimum of one year in a management role.
  • Two or more years of experience in hospice or home care, with a strong understanding of the hospice concept and terminally ill patients and their families.
  • Excellent patient assessment skills, good oral and written communication, and thorough knowledge of managed care principles and regulatory guidelines.
