About the Role

We are seeking a highly skilled and compassionate Senior Patient Care Manager to join our team at Heartland Hospice. As a key member of our leadership team, you will be responsible for overseeing the day-to-day operations of our hospice sites, ensuring the delivery of high-quality patient care, and managing a team of healthcare professionals.

Key Responsibilities
  • Manage and oversee the clinical and administrative functions of our hospice sites
  • Supervise and direct a team of healthcare professionals, including nurses, social workers, and chaplains
  • Ensure the provision of quality patient care, including assessing patient needs, developing care plans, and coordinating services
  • Manage and maintain accurate records and reports, including patient charts and operational metrics
  • Participate in on-call nursing schedule and provide direct skilled bedside nursing care as needed
Requirements
  • Registered Nurse with ASN or BSN from an accredited school of nursing
  • One year of experience as an RN, with at least three years of experience in direct patient care and one year of management experience
  • Two or more years of experience in hospice or home care
  • Current license to practice nursing in the state of employment
  • CPR Certification
  • Valid driver's license and current automobile insurance
