Position Summary
The role of the Patient Nurse Navigator is pivotal in enhancing the quality and efficiency of care provided to patients. This position commences at the point of diagnosis and continues throughout the patient's healthcare journey. The Nurse Navigator acts as a dedicated advocate for patients, ensuring they receive personalized support and access to necessary resources and services.
Key Responsibilities
- Deliver tailored education and psychosocial support to newly diagnosed patients and their families, facilitating appropriate referrals to essential services.
- Provide comprehensive information to both new and existing patients regarding their diagnosis, treatment options, support services, financial considerations, community resources, and follow-up care.
- Act as a communication bridge between patients, families, and various departments within the healthcare system.
- Oversee and coordinate new patient referrals, scheduling of necessary examinations and appointments, while identifying potential care gaps and ensuring continuity of care for patients transitioning back to their communities.
- Initiate contact with every newly diagnosed patient promptly following their diagnosis.
- Evaluate each new patient's emotional and social needs, addressing barriers to care such as insurance and transportation, and connect them with appropriate resources.
- Identify and address trends that hinder timely patient care, implementing service recovery strategies as necessary to enhance patient satisfaction.
Physical Requirements
Ability to lift items weighing up to 25 pounds. Occasional bending, kneeling, and crouching may be required. The role may involve prolonged periods of sitting or standing.
Qualifications
A Bachelor of Science in Nursing (BSN) is required; a Master of Science in Nursing (MSN) is preferred. Current licensure as a Registered Nurse in North Carolina is mandatory. Certification in the relevant specialized field must be obtained within two years of assuming the position. Strong communication skills are essential, along with 4-5 years of experience in Acute Care Nursing.
