Position Overview:
- Accountable for delivering a comprehensive array of rehabilitation services, including evaluation, treatment planning, and therapeutic interventions within a collaborative environment, adhering to professional standards and ethical practices. Demonstrate responsibility for program development, quality enhancement, problem-solving, and productivity improvement in a multidisciplinary context.
Qualifications:
- Must possess a minimum of a bachelor's degree from an accredited program (or equivalent). Active licensure in the practicing state is mandatory. Current BLS Certification is required.
Experience Required:
- Background in assisting patients with rehabilitation for disabilities or injuries across various age groups.
Key Responsibilities:
1. Exhibit and communicate a professional demeanor for patients, families, colleagues, and other stakeholders.
2. Conduct initial evaluations, select appropriate assessment tools, and determine suitable adaptive equipment/materials based on the patient's age and developmental stage, ensuring documentation is completed as per policy.
3. Formulate personalized treatment plans with measurable short and long-term objectives, directly related to functional deficits, and establish a timeline for ongoing treatment. Interventions should be tailored to assessment findings and appropriate for the patient's age and cognitive abilities.
4. Assess patient responses to treatment. Modify treatment plans based on patient progress, providing updates to patients and families regarding their status and discharge planning throughout the rehabilitation process.
5. Maintain accurate documentation of patient care, including progress and clinical recommendations, ensuring compliance with policies and procedures.
6. Educate patients and families on tasks and resources to support ongoing functional independence, involving family members in treatment and discharge planning for a safe transition home. Effectiveness of education is evaluated through feedback and satisfaction metrics.
7. Engage in professional development and continuing education activities. Participate in departmental training sessions and attend external professional growth opportunities.
8. Adhere to relevant federal and state regulations, professional standards, and organizational policies.
9. Meet established productivity benchmarks while maintaining fiscal responsibility.
