Job SummaryThe Care Coordinator/Life Skills Instructor will utilize a strengths-based, recovery-focused approach to teach adults living with serious mental illness the skills they need to reach their individual recovery goals and connect with their community.
Key Responsibilities- Develop and implement individualized recovery plans in collaboration with clients and their support networks
- Provide education and training on life skills, such as independent living, employment, and social skills
- Connect clients with community resources and services to support their recovery
- Collaborate with healthcare providers and other professionals to ensure comprehensive care
- Document client progress and outcomes in accordance with organizational standards
- Bachelor's degree in social work, psychology, counseling, or a related human service field from a college/university accredited by the US Department of Education
- Two years of directly related experience in professional and/or nonprofessional roles
- Valid driver's license, current automobile insurance, and a driving record that meets organizational guidelines and requirements
- Ability to use a private car to transport self and others
