Job Summary

We are seeking a skilled Case Manager to join our team. As a Case Manager, you will be responsible for managing a caseload of participants, providing support and guidance to help them achieve their goals and overcome challenges.

Key Responsibilities
  • Manage a caseload of participants, conducting regular meetings and assessments to identify their needs and develop individualized plans
  • Provide guidance and support to participants, helping them to overcome obstacles and achieve their goals
  • Collaborate with other staff members and community agencies to ensure participants receive comprehensive support
  • Monitor participant progress, making adjustments to plans as needed
  • Document participant interactions and progress in our database
Requirements
  • High school diploma or equivalent required; bachelor's degree in a related field preferred
  • Experience working in community corrections or a related field
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Basic computer skills and proficiency in software applications
