Nurse Case Manager

Cedars-Sinai is seeking a highly skilled Registered Nurse to join our Case Management team. As a key member of our healthcare team, you will play a vital role in coordinating patient care and ensuring seamless transitions between healthcare settings.
Key Responsibilities- Assess and plan patient care needs, developing effective care plans to meet individual health goals.
- Coordinate patient care services, including medical, psycho-social, financial, and practical needs.
- Collaborate with healthcare teams to ensure timely and safe patient discharges.
- Develop and maintain relationships with patients, families, and healthcare providers to ensure comprehensive care.
- Stay up-to-date with industry developments and best practices in case management and patient care.
- Current, unrestricted California RN license.
- Minimum 3 years of acute care nursing experience.
- At least 3 years of case management experience preferred.
- American Heart Association BLS certification required.
- Case Management Certification required within 2 years of hire.
