Accurate Home Care is seeking a skilled LPN Visit Nurse to join our Home Health team. As a key member of our team, you will provide direct nursing care to patients in their homes, working closely with our experienced care givers and RN Case Managers.
Key Responsibilities:- Complete patient visits as needed under the direction of the RN Case Manager.
- Participate in the implementation and oversight of nursing and home health aide services.
- Perform nursing skills such as phlebotomy, wound care, catheter care, medication management, and patient education.
