Job Summary
The LPN will be responsible for providing nursing care to patients in their homes, working collaboratively with the healthcare team to develop and implement individualized care plans. The ideal candidate will possess excellent communication and interpersonal skills, with the ability to work independently and as part of a team.
Responsibilities
• Provide direct patient care, including administering medications, performing wound care, and providing education to patients and families.
• Collaborate with the healthcare team to develop and implement individualized care plans.
• Conduct thorough assessments of patients' physical and emotional needs, identifying areas for improvement and developing strategies for addressing these needs.
• Maintain accurate and detailed records of patient care, including progress notes and medication administration records.
• Communicate effectively with patients, families, and healthcare team members, providing clear and concise information about patient care and treatment plans.
Requirements
• Graduate from an accredited practical nursing program as a Licensed Practical Nurse.
• Currently holds an unencumbered Licensed Practical Nurse (LPN) license with State Nursing board and/or possess multi-state licensure privileges.
• One year of professional nursing experience preferred.
