Case Manager Thurgood Marshall Position

We are seeking a skilled Case Manager to join our team at Wisconsin Community Services. The successful candidate will provide supportive, recovery-oriented interaction and monitoring of homeless individuals with substance abuse and mental illness residing in the WCS Thurgood Marshall Apartments.
Key Responsibilities- Monitor resident and guest activity to ensure safety in the Thurgood Marshall Apartments.
- Make hourly rounds to monitor the environment of the Thurgood Marshall Apartments.
- Act as a role model for residents and guests.
- Interact with residents and conduct wellness checks.
- Document all resident contacts and incidents in hard and electronic files.
- Using de-escalation techniques, intervene as necessary to prevent harmful situations or behaviors from escalating.
- Collaborate with coworkers to provide ADL assistance to residents.
- Collaborate with coworkers to facilitate daily activities and groups for residents.
- Record and relay resident inquiries to the Program Director.
- Utilize Motivational Interviewing, Trauma-Informed Care, Strengths-Based Recovery, and Harm Reduction as primary skills to enhance resident participation and success.
- Respond to general inquiries and greet guests.
- Assist with onsite meals and related documentation.
- Assist with medication reminders.
- Responsible for general housekeeping in common areas.
- Communicate incidents to the Program Director and notify the On-Call Supervisor of emergencies.
High School diploma/equivalency required. No less than 1 year of experience working with homelessness, substance abuse/mental health. Additional education may substitute for some experience. Must be able to pass an Agency background check and the Caregiver's background check.
Preferred QualificationsAbility to work in a diverse setting interacting with homeless individuals with substance abuse and mental illness. Must have good communication skills, be flexible, patient, and compassionate, and maintain confidential information. Must have the ability to work independently with minimal supervision but be a strong team player. Ability to multitask, take initiative, and apply knowledge to various circumstances.
Working ConditionsWork is performed primarily in a residential setting with individuals residing in the Harm Reduction House. Some residents may be unstable and symptomatic at times. Typical work week is Monday-Friday, 40 hours, with some flexibility during the day and evening hours. Possible weekends and holiday coverage may be necessary.
Physical demands include movement within the Harm Reduction House, going up and down stairs, and lifting/carrying 20 pounds. Staff is responsible for general upkeep in common areas and can expect to assist residents with light housekeeping duties as needed.
