WCS Healthcare Partners is seeking a skilled RN Case Manager to join our team. As a Case Manager, you will be responsible for overseeing direct support professionals in various home-care settings, ensuring the highest level of care for our patients.

Key Responsibilities:

  • Float between houses each day of the week as per required by patients' needs
  • Responsible for documentation on a regular basis
  • Ability to start/stop their day for personal reasons if/when necessary, as well as setting their own schedule on a weekly basis, within reasonable day-time hours

Requirements:

  • New York State RN License
  • Willingness to work with IDD population and be flexible with scheduling
  • OPWDD or Case Management experience required
